It starts with attention. Where our attention is directed is what our consciousness is seeing.
When our attention is mostly directed toward what we are thinking then we are paying attention to our self-created reality. When our attention is mostly directed toward our perceptions of the present, thoughtlessly, then we are paying attention to the present. The only places our attention can be directed is toward our thoughts or our perceptions. Our attention is divided between the two but mostly toward our thoughts because we are constantly thinking. Attention toward thoughts is the foundation of our self-created reality, aka the Matrix, aka civilization. While it is absolutely not correct that "we are always thinking", it is correct that we are always having thoughts. Experiencing random, or unintentionally generated thoughts are not the same thing as actually thinking
